Child Health and Disability Prevention Program (CHDP)
Many children and youth in California have unmet health needs. Through the CHDP program, eligible children and youth receive periodic preventive health assessments. Children and youth with suspected problems are then referred for diagnosis and treatment. Many health problems can be prevented or corrected, or the severity reduced, by early detection and prompt diagnosis and treatment.
▪ CHDP is a screening program that also
provides vaccinations and preventive healthcare
guidance for children.
▪ CHDP is not a treatment program.
▪ CHDP, through the CHDP Gateway Program, provides an opportunity
to access health insurance coverage for treatment of conditions
discovered on the health care exam and for ongoing routine care to
persons of low income.
A free health exam may include:

Urine and Blood Screening
TB Screening
Lead Testing

CHDP Gateway
If you do not already have Medi-Cal or Healthy Families (MC/HF), when you get a CHDP examination, an application will be sent to you. You need to fill out and return in the envelope provided.
The Medi-Cal number you receive at the doctor’s office is only good for a short period of time. Look at the expiration date!!! Your application for MC/HF must be returned prior to the expiration date.

California Treatment Program provides follow up treatment for up to 60 days after a CHDP examination for children who do not qualify for Medi-Cal or Healthy Families.
